This position will be responsible for day-to-day management of Community Health projects focused on transforming primary care delivery systems, enhancing public health and primary care integration, and improving clinical quality. Describe your experience providing support to a team or unit that requires balancing demands across multiple projects, timelines, personnel, and budgets.

This position will be responsible for managing projects within the Community Health portfolio including tracking progress toward deliverables and grant funding/budgets. Describe your experience with project management tools and tracking budgets.

This position will be responsible for cultivating and contributing to the success of partnerships with external national and federal partners for system and strategic alignment. Describe your experience in partnership management and coordination.
